We are a physician-led private practice offering comprehensive psychiatric evaluation and treatment for children, adolescents, and adults. For Pediatric and Adolescent Referrals
Helpful information to include with a referral: recent office notes, medication history, Vanderbilt rating scales, psychological or school-based testing, IEPs or 504 plans, and relevant therapy records. Patients are welcome to schedule without prior records, but documentation facilitates a more thorough initial evaluation.
For Adult Referrals
Prior psychiatric records, medication history, psychotherapy notes, and relevant medical information are helpful when available.
Peer-to-Peer Consultation
Dr. Keck-Flory welcomes direct peer-to-peer calls from referring providers to discuss cases, triage referral urgency, or clarify whether a patient is appropriate for our practice. To be connected directly, contact our office and identify yourself as a referring clinician.
To Refer a Patient
Referrals may be initiated by healthcare professionals, school personnel, patients, or families. If you're unsure whether a patient is appropriate for our practice, call us — we're happy to help you figure out the right next step.
